Supported Living
Trinity Care believe that people should be supported to be as independent as possible and that our staff encourage individuals to engage in all areas of their everyday lives, including household tasks, personal care, and making decisions about activities and experiences that they would like to take part in. We offer bespoke packages of care and support for individuals, for example, currently in a residential setting who may want to step down from formal care to living more independently.
We offer a care pathway to support individuals in a residential setting to enable a step down from formal care to living independently.
What is Supported Living?
Supported Living is a term used by health and social care professionals to describe the premiss of a person who is not able to live entirely independently living in their own home with a package of care and support which enables them to do so.
If you need some support but want to maintain as much independence as possible or need to learn how to become more independent supported living might be an option for you. To help you to live comfortably, you may need a temporary support package. If for instance, you have just been discharged from hospital. Or you may need accommodation with a long-term package in place to support you. The landlord is not the service provider, the individual has their own tenancy, which we would support them to uphold.
Available Accommodation
We currently have access to 20 apartments. On each floor there are 6/7 apartments providing supported living for individuals who maybe stepping down from a hospital, full-time residential home or residential college, or just moving out of the family home. We have access to a 2 bedroomed house and a 5 bedroomed house if you would prefer to share with friends.
